In order to achieve more ground clearance for the landing gear legs on your 5th wheel trailer, the only real option you have is to raise the trailer. The reason for this is that the more you raise the nose, the more unlevel your trailer will be which will then cause sway issues. We have our skid wheels listed by size relative to the length of the RV but at the end of the day I would go with whatever size you think would work best. In general the longer RVs like over 36 feet call for 6 inch diameter skid wheels like what comes with he Ultra Fab 6 inch wheels part # UF48-979013 which are weld on.

I have a solution for you, but the Roadmaster Dual Hitch Receiver Adapter # RM-077-4 that you referenced is only designed for flat towing vehicles so I cannot recommend it for you.
Instead you would want the Curt Multipurpose Ball Mount part # D210. This will let you tow your boat with the ball mount and then use a carrier on the top hitch opening. Though technically not light duty, I recommend going with a Draw-Tite Ultra Frame Service Body Trailer Hitch. This will have a 2" receiver opening and the cross tube will measure 44" across, so it will have no issues fitting the 41" frame on your 2006 E-450 chassis.
